The checkboxes and objective states aren't included with the objective texts.

Well, I spent a full day on this, and can't find the root cause of the problem.
I have learned the following:
1. The problem didn't exist in revision 15000.
2. The problem first appeared in revision 15005. This requires the revision 15023 executables to include a source change required by 15005.
3. The focus of revisions 14997, 15000, and 15005 is to allow text scaling on the Objectives screen, so the work in 15005 appears to have caused the problem.
4. The in-game Objectives screen doesn't have the problem (the checkboxes appear), but the Objectives screen presented after mission failure does have the problem (the checkboxes aren't there). Oddly enough, the same gui is used for both screens.
At this point, I have stopped looking at the problem (gave myself a day), and am assigning it to Obs, since he committed the original work in objective scaling. Hopefully he can find the root cause.
